Need help and information.   Looks like it is missing top.   It is for sale.

You need the right catalogue, Anthony. More than likely 30s. There are a few catalogues, but the ones I have seen I don't recall the pedestal.

What year would patent 1699201 be?

Name says Edwin F. Guth Co.

Was there a name change that might tell a before or after date?

The go to guy is Mike Kearns. Guth had a gazillion fixtures. If Mike hasn't seen it in old paper, patent design, or his piles of image files, unless someone else comes forward with one, you may have one of one to surface.

I've got #2 and I think I've seen a 3rd one somewhere in the past 10 years. Guth did produce some fabulous fixtures with & without fans. I have wondered many times if there ever was a lighting adapter, Torchier style, that could have been had to place atop that motor.

As early as 1931, there is a version with a lamp, and without. Anthony's is the version with a lamp. From Bill Hoehn:

These Guth "Portable Pedestal" fans are listed and pictured in their 1935 and 1938 catalogs.
Without lights it is R 4810;
"Portable GUTHFAN, adjustable from 5 to 7 feet high. Two speeds. Finished in English Brass (or Polished Chromium) and equipped with 18 foot cord and plug."  

With 3 lights it is R 4811;
" same pedestal and fan as R 4810, but with the advantage of the efficient Guth Super Illuminator Indirect Lighting Unit. Pedestal and base in English Brass or Polished Chromium. Reflector is Satin and Polished Aluminum."
